Out-of-Hospital Do Not Resuscitate Orders

In the state of Texas, there are Do Not Resuscitate (DNR) orders that can be honored in hospitals, and a separate form to be used in other healthcare facilities or while being transported. This is known as an Out-of-Hospital DNR (OOH-DNR). If you are sure that you do not wish to receive life-saving treatment, such as CPR, in the event of an emergency, you may wish to have an OOH-DNR in addition to the DNR your doctor or hospital has.

Like the DNR that applies when you are in the hospital or with your doctor, those with an OOH-DNR will not receive:

  • CPR
  • Artificial ventilation
  • Defibrillation
  • Cardiac shock
  • Tracheotomy or other advanced airway management

While it can be confusing to some individuals who they need more than one DNR, legislation states that there must be an OOH-DNR for patients not to receive these life-saving services if they are, for example, picked up by an ambulance or admitted to a nursing home.

In order to make it easier for people who wish to have an OOH-DNR to identify themselves, legislation allows that a copy of the OOH-DNR should be accepted in place of an original, if needed. Also, those with an OOH-DNR may get an identifying device (such as a bracelet) that instructs all medical personnel not to give life-saving measures. This is also accepted in place of the form.

It is important to know that there are a few reasons that medical professionals can refuse to honor an OOH-DNR: if the person is or appears to be pregnant, if the person only has the form, and it is filled out incorrectly, or if there is any suspicion that foul play caused the person’s current health condition.
