A Directive to Physician & Family or Surrogate gives you a voice in your end-of-life medical care. It allows you to specify your wishes, when you no longer have capacity to make decisions regarding the use of extraordinary measures, dying a natural death that is as pain free as possible, or the use of life support equipment. This type of document is particularly advisable for people who want to relieve their loved ones from being put in the position of deciding whether to “pull the plug.” Virtually all hospitals ask people to complete one of these forms before or when they check into the hospital. If you already have a Directive to Physician & Family or Surrogate, it is advisable to make sure your treating doctor has a copy on file.
